Foreign travelers eligible to travel to Kenya may need to complete a Health Surveillance online form.
Below you can find a complete guide to how to fill out a Kenya travel health certificate and how to submit the form when applying for a Kenya eTA online.
Under the Public Health Act CAP 242 of the laws of Kenya, it is now required for certain passengers to provide information to a Kenyan Port Health Officer through the submission of an online form prior to departure for the country.
The information provided on a Traveler Health Surveillance Form for Kenya is being collected as part of the public health response to the coronavirus pandemic.
Only passengers with flu-like symptoms need to complete the travel health form. All other travelers do not need to follow this step.
The Kenya travel health certificate can easily be obtained when applying for a Kenyan eTA online.
After filling out a Kenya eTA application form with personal, passport, and travel information, the applicant is directed to the payment page where they are required to enter valid credit or debit card details to pay the eTA processing fee.
On this same page, there is now an option to include the submission of a Traveler Health Surveillance Form with the online eTA application for a small extra fee.
After completing the form and submitting it along with the application, the applicant will receive a QR code once the eTA has been processed and approved. This code must be presented to port health officials upon arrival in Kenya so that the traveler may be permitted to pass through immigration.
In order to complete the online Health Surveillance form, travelers will be asked to provide the following:
Mandatory personal Information
Optional personal information
Mandatory Travel Information
Optional Travel Information
Mandatory Health Questions
Additionally, the traveler is also prompted to provide the name and contact phone number of next of kin, as well as their postal address, although this is also optional.
